    If you would like to file a formal grievance, you may write to the JP Schools 504 Coordinator. Once a written grievance is received, the District will investigate the allegations.

    The written grievance must contain:

    1. The nature of the grievance
    2. The facts upon which the grievance is based
    3. A list of all witnesses
    4. The remedy requested
    5. The complaints signature and the date the grievance is filed
