School Health Center

  • The West Jefferson High School Health Center is a comprehensive health care clinic that is housed on the West Jefferson High School campus and provides students with medical services. The goal of the Health Center is to provide accessible medical and mental health care for all students on campus. A parent must sign a written consent form for a student to receive free services. Students who have completed the consent form can receive services from licensed physicians, nurse practioners, licensed clinical social workers, or psychiatrists who can diagnose and treat physical or mental illness early and cost effectively. In order for a student to receive services , a written or electronic consent must be fully completed by a parent or guardian. There are NO OUT OF POCKET COSTS for any student, insured or uninsured.

    All students must have a current consent and enrollment form on file to be seen at the West Jefferson School Health Center. Consent and enrollment forms are sent home at the beginning of the year, but parents or guardians may sign their child up for services at any time during the school year. Electronic consent forms may also be found below of this web page through a QR code or links. Paper consent forms can also be found and printed. Links are provided below for the forms.

    Co-payments are waived for all students enrolled at SBHC. Commercial insurance, including Medicare, is accepted. Students who are uninsured can also receive care with a consent and enrollment form on file with the health care center. Services are given to all students at NO COST. 

    SERVICES OFFERED:
    Primary Care - well & sick visits, immunizations, sports physicals, acute &  diagnostic care
    Behavioral Health-individual & group therapy including: stress & anger management, depression,
    trauma, bullying and relationships.
